Your proposed modifications sound okay. Your 3rd listed modification would be a part of level modification and not something on its own. Keep in mind that each unique level can be counted as a significant modification. An instructions page that is simply text will not be enough for a full significant modification. Be sure to do a little more to make your instructions view unique, such as a graphical show of the controls.
